Role Summary: The post-holder is responsible to the Manager of the Specialist outreach recovery team for the assessment of recovery needs, development, implementation and evaluation of care for patients with a primary diagnosis of complex psychosis. The practitioner is responsible for assessment of recovery needs, development, implementation and evaluation of care, within agreed personalised care procedures. The post holder may be required to supervise junior staff within SORT. Accountability: The post holder is accountable to the SORT Manager through the Line Management system within SORT Pay & Conditions: The post holder must have completed training as a first level registered nurse in mental health with a currently valid registration with the Nursing and Midwifery Council. Salary will be Band 5. Key Tasks and Responsibilities of the Post: The post holder will undertake assessments as part of the MDT routinely for all new referrals as appropriate, at home or in a clinic setting, in accordance with the agreed Operational Policy. The post holder will provide nursing care and recovery interventions as appropriate in the right setting, in accordance with the agreed Operational Policy. The post holder will provide community based recovery support for a defined group of patients within the City. The post holder will work closely with all other clinical professionals e.g. Consultant Psychiatrist, Clinical Psychologist, Social Workers, Occupational Therapist both within SORT as well as liaising with other Professionals externally where appropriate. The post holder will act as a key worker for a defined group of practice patients and organise care in accordance with the personalised care framework. The post holder will be expected to attend and contribute to weekly MDT meetings, daily morning meetings and caseload formulation sessions within the team. The post holder will support senior members of the team with day to day management of the team and medication administration. The post holder will be responsible for supporting the team with improving quality, and will be expected to attend training provided in house as well as mandatory training to maintain high standards of knowledge and skills. The post holder will be expected to undertake a clinical lead for the team in either an area of interest to the post holder, or an allocated lead depending on the needs of the service. The post holder will ensure that their clinical practice is based on evidence and good practice and in accordance with any NICE guidance. The post holder will be individually accountable for their practice in accordance with the NMC professional code of conduct. Staff Management: The post holder may be responsible for supervising some junior staff within SORT. Liaison and Administration: The post holder should maintain close links with in-patient services at Glenbourne recovery inpatient services, 3rd sector organisations, Social Care, Dual Diagnosis Services. The post holder will keep up to date electronic/paper records of all patient and staff activity, ensuring these are kept in a safe place, in accordance with NMC guidelines, and Livewell Southwest policies including Data Protection Act and the Mental Health Act. The post holder will be aware of NMC and Livewell Southwest guidelines on record keeping, confidentiality, Mental Health Act and Data Protection Act. Professional Development and Education: The post holder will be required to keep up-to-date this will include Mandatory training and developments in mental health nursing or sphere of practice. The post holder will participate in the provision of relevant community placements and training programmes for nurses. The post holder will be required to ensure that they receive appropriate regular line management and clinical supervision in line with current policies of Livewell Southwest.
