We are seeking a newly qualified Nurse Associate who is positive, compassionate, and self-motivated to join our team at Creswell and Langwith Medical Centre. This position is supported by the PCN ARRS funding initiative and comes with a 12-month fixed-term contract, which may be extended depending on NHS funding and guidance.

In addition to fulfilling the responsibilities of the Nurse Associate role, the successful candidate will also be expected to dedicate an appropriate portion of their time to facilitating and participating in monthly MDT meetings . As a key member of the CST multidisciplinary team, you will collaborate closely with the clinical staff as well as other services—including the social prescribing team, occupational therapy, and Adult Social Services—to ensure that vulnerable patients receive appropriate and effective care.

Strong communication skills are essential for the Care Coordinator role. To support this, the PCN provide a two-day health coaching skills course that will equip you with the necessary skills for this aspect of the job.

It is Trust policy that for posts that are for a time limited period, applicants who are currently employed by an NHS organisation will only be appointed on a secondment basis and should have approval from their line manager this is possible before applying. Applicants who are not employed by an NHS organisation, will receive a fixed term contract.

· Nursing Associates work in practice, independently under the leadership of registered nurses.

· Provide compassionate, safe and effective care and support to patients.

· Provide effective evidence-based care for service users under the supervision of registered nurses to perform non-complex activities, in line with clinical governance and professional standards.

· Perform routine nursing tasks such as venepuncture, BP checks, ECG, BMI, lifestyle advice, CVD health checks, screening and long-term condition annual reviews as directed and agreed with line manager, registered general nurses and GPs.

• Actively participate in MDT meetings as required

· · Support the delivery of services to meet QOF, enhanced services specifications, and NHSE contract changes

· · Administer medications as prescribed and in accordance with NMC standards under Patient Specific Directions. [Immunisation and Vaccination]

· Maintain accurate, contemporaneous patient records in line with Trust policy and the NMC Code.

· Following agreed protocols with referral to Practice Nurse or GPs as appropriate to promote health and well-being and prevent adverse effects on health and well-being.
