Summary

Trinity Medical Centre, Hove is recruiting for an ADVANCED NURSE PRACTITIONER (ANP) We are happy to look flexibly at options and hours that will suit the individual. Our ideal is 15-18 hours a week. Nurse-led management of acute undifferentiated and undiagnosed conditions includes minor illness, minor ailments and injuries Experience in managing these patients in the community Recent primary and/or urgent care experience nursing qualifications RGN Recent experience of face-to-face patient care including assessments of those housebound (mostly frail) in their own homes or care & nursing home settings Experience of those who have complex physical health needs requiring assessment of acute need but taking into account advanced care plan wishes and needs Experience of assessing the housebound +/- frail patients and discussing their advanced care plan wishes within the remit of RESPECT planning Experience of completing RESPECT forms would be beneficial, but not essential Degree level / Postgraduate level ANP/Specialist qualification or equivalent Evidence of recent study Recognised Independent Nurse Prescribing Qualification Come and join our dynamic and cohesive team in a fabulous building in central Hove. Close to excellent transport links and with on-site parking. We have come together from a merger of three local practices. We consult using EMIS Web and have been pioneers for electronic prescribing and workflow document management. We are a training practice with excellent QOF and we host GP Speciality Trainees as well as FY2s and medical students from BSMS Our current list size is 25,000 patients. Our team of 17 GPs is supported by our ANP Team as well as a number of Allied Health Care Professionals including pharmacists and paramedics and our fantastic nursing team run all chronic disease clinics to provide excellent care to our local community. We run specialist clinics - joint injections; minor ops; family planning and community anti-coagulation. We have a really great working environment and a very friendly team who meet for coffee daily and will often go out socially as a team. You will be well supported by our GP team.
