Are you passionate about improving care for people with learning disabilities and autism?

Do you have strong coordination and communication skills? If so, we want you to join our team as aDSR Coordinator.

This is an exciting opportunity to play a key role in developing and delivering theDynamic Support Register (DSR)across Birmingham and Solihull. You’ll help services/providers identify individuals at risk of hospital admission and ensure they receive the right care in the community. Working closely with multi-agency partners, you’ll keep the DSR live, accurate, and impactful.

What we’re looking for:
• Degree-level qualification in health/care or equivalent experience
• Experience in multi-agency working and healthcare for people with autism/learning disabilities
• Strong organisational, data analysis, and ICT skills
• Ability to work autonomously and manage complex situations

What you’ll do:
• Manage referrals and monitor BSOL Citizens on the DSR
• Coordinate multi-agency meetings and responses
• Provide education and training on DSR processes
• Support national reporting requirements and local transformation programmes
• Act as a specialist in DSR pathways, embedding best practice
