Crynodeb
The post holder provides clinical leadership and support to the community nursing teams. This involves managing a wide range of complex and sometimes conflicting priorities.
Implementation of and compliance with policies and procedures, including safe staffing across the outpatient and community services ensuring the delivery of high value, family centred responsive and efficient patient care.
Facilitate and provide the delivery of a high standard of patient care in conjunction with the child and their family/carers.
Support and develop family centred care.
Promote interagency multidisciplinary working by developing joint action plans, protocols, and pathways to ensure continuous quality improvement for service needs.
At all times ensure that the needs of the patient are placed at the centre of care delivery.
Lead and motivate the team, supporting with prioritising caseloads and self management.
To support all team members to teach and assess, support the wider nursing team to families/carers to carry out specific nursing care for a variety of conditions.
Working in collaboration with Parents/carers, acting as an advocate for the families, providing care to children and young people in accordance with the child’s plan of care to meet the nursing and therapy needs, and to liaise with all members of the multidisciplinary team when changes occur.

Take responsibility on behalf of the Clinical Service Manager for the operational management and continuity of the childrens community nursing service.
Provide leadership to and be responsible for team members in the day to day running of the service, promoting multidisciplinary working and ensuring organisational and divisional barriers do not prevent the development and functioning of integrated care pathways and high quality patient care.
Undertake regular audit of adherence to associated local and national policies, guidance (e.g. NICE) and standards of care
Contribute to the achievement of standards within the areas of responsibility, working effectively with other service managers and clinical staff to ensure agreed standards are maintained
Prepare and deliver service reviews as agreed, reporting on the performance of services within area of responsibility.
Support service to ensure effective leadership and management of resources, including budgetary responsibilities
Investigate and respond to complaints and incidents/serious incidents within a specified timeframe, analysing and monitoring data collected and collated from each service area to ensure that action is taken to correct and prevent recurrence.
Ensure escalation processes are understood and embedded within operational teams.
Be responsible, in conjunction with the Clinical Service Manager and Finance lead, for ensuring the budget remains in balance, efficient use is made of service resources and the Trusts Standing Financial Instructions are adhered to.
• Take responsibility for performance management and achievement in the service, including management of effective clinical governance and enhancing patient safety
• Registered Sick Children's Nurse-RSCN/RN15
• Clinical qualification to first degree level or equivalent
• Evidence of continuing professional and managerial development
• Knowledge and expertise in management acquired through in depth experience or theoretical study, equivalent to post graduate level.
• Practice teacher
• Extensive Community Experience
• Knowledge and understanding of NHS and social care policy and directives (as relevant)
• Significant experience in an NHS Clinical Service management/leadership role.
• Knowledge of Community Children's Nursing/Special Schools
• Knowledge of current safeguarding procedures
