Crynodeb
To act as the patients key worker, developing a professional relationship and to provide support at diagnosis through to treatment and beyond to survivorship or end-of-life. To provide professional advice on Haematological issues and be a resource of clinical expertise for other healthcare professionals within the Trust. To co-ordinate the patient's care pathway and promote personalised care. To contribute to service development in regard to the Haematology nursing service. To liaise with Multi-disciplinary colleagues to ensure best practice and safe and high quality care is provided.
