We are excited to offer an excellent opportunity for an experienced Occupational Therapist or Physiotherapist to join us as a Team Leader within our integrated Community Therapy Team, serving the South Lakes and Furness areas. This role provides the chance to lead a dedicated, multidisciplinary team, supporting the delivery of high-quality, person-centred care to our local communities.
As a Team Leader, you will:
• Support Community Therapy and Rehabilitation Services, ensuring safe and effective care for patients.
• Lead and overseeTherapy Discharge pathways, promoting smooth transitions from hospital to home.
• Champion theWellness Model, focusing on holistic, person-centred care that empowers independence and recovery.
• Manage theday-to-day running of the service, working collaboratively with a team of Band 7 Clinical Leads and a fellow Joint Team Lead.
• Ensureservice delivery standardsare met, monitoring performance and driving continuous improvement.
• Work closely withsenior leaders, receiving guidance and support to achieve operational and strategic goals.
• Provide leadership, mentorship, and professional development opportunities for team members.
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
