Main Duties and Responsibilities Give a high standard of specialist nursing care to those people referred to the learning disability team and to assist them to attain maximumindependence and quality of life by; assessing, developing, implementingand evaluating programmes of care and Health Action Plans in conjunction with service users / carers. Offer support to families and service users in order for them to gainmaximum benefit from available services, resources etc. Take responsibility for the management of a caseload. Undertake a range of assessments in conjunction with service users,carers / families, using a person centred approach and identify andsupport Health Facilitators for people with learning disabilities inaccordance with the recommendations of Valuing People A NewStrategy for Learning Disability for the 21st Century.AFC 23/05/17 JME-106-17 Community LD Nurse Band 5 Provide a quality service in line with service philosophy and the principles of Valuing People A new strategy for Learning Disability for the 21st Century. Contribute to the Risk Management and Critical Incident Analysis reporting processes. Be involved in the setting, monitoring, maintaining and auditing ofstandards. Patient Care The post holder will be required to deliver a range of teaching sessions to individuals and carers of people with a learning disability. Support people with learning disabilities who may exhibit a range ofchallenging behaviours, which may include verbal and/or physicalaggression. Devise in conjunction with carers, families and other professionals,appropriate strategies to deal sensitively with the difficult to managebehaviours. To provide an efficient, timely, and high quality community nursing service to people with a learning disability and complex needs. This requires a specialist knowledge base on which to practice. To develop and evaluate a range of interventions and care plans. The post holder will take an active role in identifying and managing risk. To provide comprehensive specialist assessments to individuals with alearning disability. To work within a multi-disciplinary team to achieve the best outcomes for the individual with a learning disability. Education To work in accordance with Pennine Care Learning Disabilities policiesand procedures. Work in partnership with all members of the learning disabilities team to achieve Good Health objectives of Valuing People. To work collaboratively with primary care and secondary care services to ensure the health needs of people with learning disabilities are met. Attend study days and in-service courses as required. Identify own training needs with assistance from Line Manager. Take personal responsibility for being professionally up-to-date. Be involved in the education of the community about people with learning disabilities. Participate in training programmes. Educate service users, carers and others involved in their care, including Health Action Planning, Health Facilitation, Learning Disability Awareness and Challenging Behaviour.The post holder must comply with and support the development of theperformance standards within the service/department to ensure theservice is responsive to and meets the needs of its customers.The post holder will be required to develop and maintain good workingrelationships with all patients, service users, staff, contractors andwhere appropriate, members of the public.The Trust aims to maintain the good will and confidence of its own staff,patients, service users, NHS contractors and the general public.
