Crynodeb
Carry out clinical care interventions daily for children and young people with healthcare needs in the special school/nursery setting To understand their own scope of practice and limitations and refer appropriately as required. To be competent and skilled in clinical competencies relevant to the children and young people health and welfare and identified in their HCP/ EHC plan. Follow the SHC Services and PAN London Safeguarding Childrens Board procedures to safeguard children. To be actively involved in SHC clinical governance processes including the clinical audit process and risk management and patient safety. To promote positive health by assisting children and young people to be as functionally independent and self-caring as possible and to be actively involved in their own health care. Contribute to and undertake health assessments to identify the health needs of individual children, implementing and evaluating programmes of care, referring as necessary to other professionals Promote the mental, physical and emotional health of children and young people with physical and learning disabilities liaising with parents/carers, teachers and the multidisciplinary team To have a practical understanding of the implications of providing individualised healthcare support within a school setting To promote equality and respect for all children, young people and staff. To promote and contribute to the provision of advocacy for children and young people in special schools. To respond in a timely manner to information received from other professionals following the IG and information sharing policies.
