Crynodeb
Communication Communicate clearly and professionally across the MDT, providing excellent service in line with Trust values. Handle enquiries sensitively, escalate appropriately, and share information to support service improvement. Analytical & Planning Accurately update patient and activity records, maintain SSNAP data quality, run reports, and resolve data issues. Manage electronic patient records, support meetings, liaise with regional stroke teams, and provide cross-cover as needed. Leadership Work independently using initiative and sound judgement. Manage and prioritise workload, maintain confidentiality, and keep the Service Manager and SSNAP Lead informed of progress, risks, and delays. Data Quality & Audit Produce reports, analyse patient records, support audits and research, and ensure accurate coding and data interpretation. Training Complete mandatory training and appraisal, support new staff, and assist with implementing new policies and procedures.
