Demonstrate the knowledge and skills to articulate comprehensive assessment, formulation and care planning, verbally and in written form. Act with inclusivity, initiative and creativity to incorporate strength-based principles to support people to address their individual needs. Communicate specialist condition-related information to the person, carers and other health professionals; where there may be significant barriers to acceptance or understanding. Uses communication, negotiation, conflict management skills; requiring empathy, reassurance and persuasion, where there are significant barriers to change. Demonstrates a comprehensive critical awareness of the potential for stigmatisation and the possible impact on co-production, intervention and motivation. Demonstrates a depth of understanding around the impact of population health and wellbeing, taking appropriate steps to address inequalities and vulnerabilities. Demonstrates a critical understanding of the multiple perspectives within a situation; ensuring the needs of carers, of all ages, are appropriately assessed, explored and addressed. Provide ADHD diagnosis support/queries/training: Offer individual advice and support/signpost families, post diagnosis on the management of ADHD and behavioural difficulties. Medication Review (booked and adhoc): including a health assessment in association with medication and offer advice and support around medication and repeat prescribing as appropriate. Prescribing: To act as an independent non-medical prescriber ensuring full adherence to scope of practice, supervision and competency requirements Liaise with partner agencies, including Education and CAMHS/community paediatrics for updates and support for the children/young people and their families. Shared care and partnership: Liaise with the primary health care team regarding referrals and management of CYP with ADHD and behavioural difficulties. Education and Training: Liaise with SENCOs on appropriate support materials for children and young people and families, including resources for use within schools. Transition support: Provide advice and support for the young person transitioning to adult/primary care services. Demonstrate a deep and systematic knowledge and understanding of local care systems (including agencies outside of statutory care); collaborating with and referring to them independently and when required. Act as clinical lead on the delivery of specialist-specific interventions, working in collaboration with the young person, their family and carers, as well as a range of professionals and agencies. Hold direct line management responsibility for the ND Wellbeing Practitioners within the CYP ND Hub, with responsibility for the delivery of highly effective staff management, supervision and support for these staff. Demonstrate support, respect and courtesy towards colleagues, working in a professional manner and with Trust values always in mind, respecting the boundaries of the relationship with the CYP, families, carers and staff. Challenges, where appropriate, professional and organisational boundaries, identify areas for skill/knowledge development and apply these to practice ensuring continuity and high standards of care. Exemplify leadership, resilience and determination, manage situations that are unfamiliar, uncertain, complex or unpredictable and seek to build confidence in others. Receives, leads, delivers and exemplifies a culture of critically reflective clinical supervision. Manages the expectations of both clinical interventions and service demands in knowledge of available resources (i.e. finance, staffing and access to services). Supports the development of trainee Advanced Nurse Practitioners, acting as a role model, supervisor and assessor, where required. To effectively manage own workload and the need to frequently re-prioritise work to respond to multiple, competing demands.
