Job Purpose

The Team Leader is responsible for leading and managing care staff on shift to ensure the delivery of high-quality, safe, and person-centred care to residents. The role combines hands-on care delivery with leadership responsibilities, including shift management, staff supervision, medication oversight, and ensuring compliance with regulatory and organisational standards. The Team Leader plays a key role in promoting residents’ wellbeing, dignity, and independence while supporting and developing the care team.

Key Responsibilities

Shift Leadership and Care Delivery

Lead and manage all aspects of the shift, allocating duties appropriately and ensuring adequate staffing levels to meet residents’ needs.

Provide hands-on care as part of the rota, role-modelling best practice and supporting Care Assistants in delivering person-centred care.

Ensure care is delivered in line with individual care plans, risk assessments, and agreed outcomes.

Medication and Clinical Oversight

Administer, record, and order medication in line with policies and training.

Ensure medication errors or concerns are acted upon promptly and escalated appropriately.

Monitor residents’ health and wellbeing, reporting any changes to senior staff or healthcare professionals.

Care Planning and Reviews

Review residents’ care needs regularly, ensuring care plans are accurate, up to date, and reflective of changing needs.

Analyse trends and observations to support continuous improvement in care delivery.

Support residents’ nutritional needs, monitoring intake, hydration, and changes in weight or BMI.

Staff Supervision and Development

Support the induction, supervision, coaching, and ongoing development of Care Assistants.

Address performance concerns in a timely, supportive, and professional manner.

Promote good communication, teamwork, and staff engagement across shifts.

Safeguarding, Compliance, and Quality

Ensure compliance with Care Quality Commission (CQC) Fundamental Standards, safeguarding procedures, and relevant legislation.

Identify and report any concerns regarding poor practice, safeguarding, or health and safety issues in line with policy.

Maintain accurate records, documentation, and reports, ensuring confidentiality and data protection at all times.

Communication and Multi-Agency Working

Communicate effectively with residents, families, colleagues, and external professionals.

Liaise with health and social care partners, supporting continuity of care and positive outcomes for residents.

Escalate concerns or operational issues to the Deputy Manager or Registered Manager as required.

Person Specification

Essential:

Experience working in a residential care setting with older people or adults with care needs.

Strong understanding of person-centred care, safeguarding, and health and safety.

Ability to lead, motivate, and support a care team.

Good written and verbal communication skills.

Desirable:

NVQ / Diploma Level 3 in Health & Social Care or equivalent.

Previous experience in a supervisory or team-leading role.

Knowledge of CQC standards, the Mental Capacity Act, and Deprivation of Liberty Safeguards.
