To provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence and quality of life. Be accountable for own practice and take every reasonable opportunity to sustain and improve knowledge and professional competence. Closely working with patients/carers and professionals, providing support to mental health services service users. Attend and co-ordinate CPA meetings, review meetings and planning meetings where appropriate. Providing and oversee 7 day follow up post presentation to acute hospitals, working with families/carers and networks Prompt presence providing specialist nursing assessment to mental health presentations where indicated Develop strong links and relationships with other teams and Allied agencies Before submitting your application, please ensure you have the correct right to work.
