Please note, at this stage, the role is open only to internal staff (including NHSP and Agency staff currently on placement within our organisation) and staff substantively employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Any individual who is at risk of redundancy will be given priority, provided they meet the essential criteria.
We are looking for a Band 7 Practitioner to join our team based at Priority House in the Home treatment team.
The role includes clinical Leadership of a multidisciplinary team working within the Home Treatment Team and Rapid Response service.
Ensuring high quality care is delivered to all clients on the caseload, who are experiencing Mental Health Crises.
The post-holder is supernumerary, working clinically to support junior staff with complex interventions and presentations.
• Clinical leadership
• Sharing expertise in mental health crises in older adults.
• Collaborating with Acute Trust and KMPT teams to ensure effective care.
• Identifying social needs and coordinating care plans.
• Managing risk and safeguarding concerns.
• Developing integrated care plans and involving carers appropriately.
• Acting autonomously in assessments and care planning.
• Supporting service development, staff supervision, inductions, and clinical audits.
• Promoting equality, trust values, and cost-effective resource use.
• Driving service improvements through PREM & CREM feedback.
