Crynodeb
To manage and oversee the social visitor accreditation process through liaison with social workers, multi-disciplinary teams and security staff in line with visiting procedures and ensuring this is communicated to relevant departments. To act as Ward Carer Link for a particular care stream as part of the Triangle of Care (TOC) agenda providing 1:1 support, organise and facilitate any staff group sessions aiding with TOC assessments, carer information and attend ward community meetings. To improve patient and carer experience of the ward and enhance carers/ ward staff communication. To support and facilitate onsite visits between social visitors and patients often being the sole member of staff on those visits. This includes leading on onsite child and virtual visits. To organise and book any interpreters required for any social visit. To provide the technical instruction / equipment or assistance to ward staff if required for MS Teams Interpreters. To navigate clinical systems (RIO) to gain relevant patient information (mental/physical/risk/ historical and current issues prior to social visits) & to feedback and report on patient / carer contact. To liaise with social workers and ward staff prior to supporting any visit and feedback to the wider MDT (Multi-Disciplinary Team) about visits or issues raised in carer contact via the Trust wide RIO patient recording system. To maintain accurate, timely and relevant clinical records. To maintain the safe custody of healthcare records or other confidential information. To support and be on the ward if we are there to release a ward staff member to enable an onsite or purple visit to go ahead. To assist organising and supporting social visitor meetings and involvement events at Rampton Hospital. To develop and ensure carer involvement. Support planned collaborative events at specific points of the year (e.g. National Volunteering Week, Mental Health Awareness Week, National Carers Week, patient Oscar events etc) to share and celebrate the positive contribution volunteers and Carers make to our organisation, our services and to the lives of patients. To support ward staff in the implementation the patient / carer communication plans. To support relevant carer awareness training sessions to staff as required. To maintain safeguarding responsibilities for children and vulnerable adults in line with Trust policy, and Local Authority procedure and guidance, including making timely child and adult safeguarding referrals, as required. NB: this is not an exhaustive list of duties and may be changed in line with service needs in conjunction with the post holder. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Please note applicants will be required to pay for their DBS check.Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
