Adult Social Care Advisor
The role is offered on a fixed-term basis until March 2028, with any extension dependent on securing further funding.

To help us make this happen, we’re looking for an Adult Social care advisor
Our See & Solve Team is the first point of contact for adults, families and carers seeking advice, support and guidance. We are looking for a compassionate and resourceful Social Care Advisor to join us in delivering truly person‑centred, solution‑focused conversations that help people stay in control of their lives, based within Active Hull
As a Social Care Advisor, you’ll work within a multidisciplinary environment, using skilled conversations to help people identify their strengths, set their own outcomes, and explore options to maintain independence.
You will:

Provide timely, meaningful, strengths‑based conversations that empower people to shape their own support.
Offer guidance, information and advice using tools such as Connect to Support and signposting to universal, community and voluntary services.
Support people to access physical activity to improve health and social outcomes for people
Support people to develop informal support networks and connect with their local communities.
Recognise safeguarding concerns, promote safe decision‑making, and follow “Making Safeguarding Personal”
Work flexibly across communication channels; phone, face‑to‑face, digital, sometimes in fast‑paced or emotionally demanding situations.
Collaborate with a wide range of professionals and partner agencies to ensure people receive the right help at the right time.
Record accurate and high‑quality case information on internal systems.

This is a role where no two days are the same and where the work you do will have a direct and meaningful impact on people across Hull.
You will be someone who is:

Warm, empathetic and able to build trust quickly.
Skilled at having solution‑focused, empowering conversations.
Confident supporting vulnerable adults, including those in distress or crisis.
Able to work under pressure, manage your own time and respond to changing demands.
A strong communicator, both verbally and in writing, with excellent interpersonal skills.
Committed to co‑production, dignity, independence and person‑centred practice.
If you have experience working with vulnerable adults (professionally, personally or in a voluntary capacity), we want you on our team.
