Clinical Under the direction of senior clinician add patient care plans Use electronic systems to input and access information as required by the senior clinician To receive and gather complex sensitive information and accurately document the information Under the supervision of senior clinician undertake communication to identify the needs of the patient or referrer to ensure all information is gathered, documented and forwarded on using the correct pathway following ECCH process Liaise directly with social care colleagues to ensure appropriate patient response Under the supervision of senior clinician conduct patient deferred visit calls where appropriate Liaise with professional colleagues both within the team / organisation and with external partners Recognising the extent of their competency and identifying when referral to a clinician is required in accordance with agreed standard operating procedures Undertake calls to inform patients of deferred visits Professional Be able to communicate with patients / clients / relatives Having an excellent telephone manner, holding the PCH triage phone Monitor PCH Monthly planner and caseloads to ensure all visits are allocated, identifying any patients that may be missed Data quality check waiting lists Conduct the Lone worker checks for the PCH Monitor and follow up on PCH equipment tasks Communicate positively and effectively Be adaptable, work with integrity and be trustworthy Recording details of all contacts with patients and other professionals using SystmOne (S1) as appropriate Ensuring that follow up actions from calls, emails, correspondence, referrals and other contacts are progressed efficiently and effectively Organisational Actively give and receive feedback in a constructive manner Support data collection and audit Work as part of a team Understanding of multidisciplinary and integrated working Own their individual responsibilities and develop team objectives To participate in staff appraisal Contribute to creating improvement, transformation and innovations of services Contribute to creating a work environment that is marked by pride, enthusiasm and collaboration Actively contribute to improving the health and well-being of the populations that is served Contribute to financial performance and target delivery Signposting of callers onto other appropriate services as directed All roles within East Coast Community Healthcare CIC (ECCH) require staff to demonstrate our Values and Signature Behaviours in the care and service they provide to patients, service users, stakeholders and colleagues.
