Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Teaching Hospitals NHS Foundation Trust plan to integrate and form a new organisation on the 1st April 2026. As such, if your start date is on or after 1st April 2026, you will be employed by the newly formed organisation. The post holder will be required to support the existing Tissue Viability Nurses in developing a research based approach to Tissue Viability. Exploring the cost effectiveness of Current treatments and the introduction of new products for the benefit of patient care. Acts as an autonomous practitioner within agreed protocols guided by organisational policies and within Scope of Professional Practice. To have an in-depth knowledge and clinical experience in the area of Tissue Viability. To support the Clinical Nurse Specialist in the development and implementation of evidence based practice and to act as a resource, because of his/her in-depth knowledge and skill in tissue viability. To collaborate with the Clinical Nurse Specialist in the implementation of educational programmes to ensure that patients within the CHS receive the best possible and cost effective Tissue Viability care. Act as a mentor to pre and post registration nurses, allied health professional and unqualified students. Duties and Responsibilities: To support the Clinical Nurse Specialist for Tissue Viability in providing an efficient and effective high quality service Maintain a high standard of patient care in accordance with NMC Code of Professional Conduct and within Scope of Professional Practice. Maintain own clinical and professional expertise by participating in clinical supervision, professional development planning and within NMC Code of Conduct and Scope of Professional Practice. Promote the clinical effectiveness of the nurse-led service using a multi- disciplinary team approach. Contribute to the development of the Tissue Viability service. Promote a culture which utilizes latest evidence and best practice. Works autonomously under indirect supervision liaising with team leaders and co-ordinators as required. Works in a flexible way to support and maintain good team working practices. Utilises analytical and judgemental skills in the assessment of patients presenting with undifferentiated and undiagnosed health problems. Provide professional advice and leadership in tissue viability To support the Clinical Nurse Specialist for Tissue Viability in the delivery of education programmes. To provide effective clinical advice and assistance in the assessment management, treatment and prevention of chronic wounds. Participate in research activity within the Tissue Viability Service. To work autonomously under indirect supervision of the Clinical Nurse Specialist Tissue Viability product evaluation to supporting informed choice when reviewing the Trust Dressings Formulary. To respect confidential information obtained during the course of your work and refrain from discussing such information without the consent of the patient/client or a person entitled to act on his/her behalf, except where disclosure is required by law or by the order of the court or is necessary in the public interest. Managerial To assist the Clinical Nurse Specialist in providing accurate data for audit, governance and service development. Demonstrate commitment to multi-disciplinary team working across the local health economy. Contribute to the development, implementation and monitoring of policies, procedures and clinical guidelines as appropriate. To act as a role model highlighting and disseminating good practice within Tissue Viability service. To work autonomously under indirect supervision of the Clinical nurse specialist in risk assessments for all patients in the community setting, ensuring the correct equipment is provided. To work autonomously under indirect supervision of the Clinical Nurse Specialist in clinical audit recording and reporting data as required by the CHS. Leadership To represent the Clinical nurse Specialist at multi agency and multi disciplinary forums within there area of expertise. To accept delegated responsibilities from the Clinical nurse Specialist within there scope of professional practice. To work autonomously under indirect supervision of the Clinical Nurse specialist in the management of the specialist caseloads. Clinical To perform specialist assessments, planning, implementation and evaluation of nursing care needs of the individual and their family/carer. To act as a resource to nursing and care homes on the treatment of chronic wounds and the use of pressure relieving equipment. To provide quality researched based nursing care at all times adhering to NMC Code of Professional Conduct. To ensure practice is evidence based and to assist the Clinical Nurse Specialist in monitoring standards of care for Tissue Viability patients provided by community nurses. To establish and maintain effective communication with patients, carers and professionals across health and social services. To ensure comprehensive documented assessment and care planning for patients with chronic wounds, in conjunction with the named community nurse and patients. To raise awareness of ethical / legal issues relating to wound management and reflect this in clinical practice To work to acquire nurse prescribing status. To enhance knowledge of tissue viability in the wider nursing and allied health workforce to support the development of best practice. Training and Education Develop and maintain own clinical and professional expertise by participation in clinical supervision, personal development planning and within the Scope of Professional Practice and NMC Code of Conduct. To participate in the identification of education needs and, implement and evaluate programmes involving all qualified and unqualified staff. To participate the Clinical Nurse Specialist for Tissue Viability in the delivery of education programmes. To maintain a high level of highly developed specialist knowledge underpinned by theory and practice. To participate in the organisation of link meetings for the purpose of regularly updating key staff on development in Tissue Viability so that they may cascade knowledge back to their colleagues. To assist the Clinical Nurse Specialist in addressing specific health targets. Contribute to and disseminate best practice through evidence, clinical benchmarking and guidelines. To take an active role in research and development following agreed trust protocols. To liaise with research departments and institutes when applicable. To be responsible for own professional development in accordance with NMC guidelines. To initiate new research projects under the supervision of the Clinical Nurse Specialist for Tissue Viability. To participate with the Clinical Nurse Specialist in the provision of accurate data as requested by the CHS. To participate with the Clinical Nurse Specialist in clinical audit, benchmarking and pilot projects in order to improve effectiveness of patient care To be responsible for the critical appraisal and dissemination of current relevant research, and lead in developing research based practice in all aspects of wound care. To work autonomously under direct supervision of the Clinical Nurse Specialist Tissue Viability with the assessment and evaluation of wound care products in order to facilitate cost effective quality care. Communication To develop partnerships with other agencies to enhance service delivery. To establish and maintain working arrangements and communication with GPs PHCT members and other services locally To represent the Clinical Nurse Specialist at multi agency and multi disciplinary forums. To attend multi agency case conferences to plan care and review existing care packages.
