Role Overview

As a Reablement Care Worker, you will support service users to achieve their personal reablement goals, working collaboratively with health professionals and the wider CRT team. You will deliver outcome-focused care in line with an individual Service Delivery Plan, supporting people to regain daily living skills rather than creating long-term dependency.

You will work to a rota and receive daily task allocations via a mobile working device, enabling efficient and responsive service delivery in the community.

Multiple posts are available, offering a range of rotas and working patterns.

Key Responsibilities

Provide reablement-focused care and support to adults in their own homes, promoting independence and recovery.

Support service users to achieve agreed reablement and wellbeing goals, in line with their assessment and care plan.

Assist with personal care, mobility, daily living activities, and confidence-building, encouraging individuals to do as much as possible for themselves.

Work collaboratively with health professionals and multidisciplinary teams to deliver coordinated care.

Accurately observe, record, and report changes in service users’ needs, progress, or risks using digital systems.

Follow service delivery plans and contribute to reviews and reassessments as required.

Work flexibly across a rota, including evenings and weekends, to meet service demand.

Maintain professionalism, dignity, and respect in all interactions with service users and their families.

Comply with safeguarding, health and safety, and data protection requirements at all times.

What We Are Looking For
Essential

A caring, compassionate approach with a strong commitment to supporting independence.

Good observation, recording, and communication skills.

Ability to work flexibly in a dynamic and fast-changing service.

Willingness and ability to undertake training to achieve the required Health and Social Care (HSC) qualification for the role.

Ability to work effectively as part of a team and independently in the community.

Desirable

Ability to speak Welsh and/or other community languages.

Previous experience in care, reablement, health, or community support services.

Training & Development

Full induction and ongoing training provided.

Support to achieve relevant Health and Social Care qualifications.

Regular supervision and professional development opportunities.

Additional Information

This post is subject to an Enhanced Disclosure and Barring Service (DBS) check.

Registration with Social Care Wales is required.
