We are looking for an Assistant Psychologist to support our discharge pathway project for an initial period of 2 months. The main responsibilities of this role will include meeting with patients for social prescribing, completing final routine outcome measures, writing discharge letters, assessing risk before discharge from community CAMHS with the support of the MDT.
The service has secured funding until April 2026 to trial our discharge pathway project. The project will allow more timely and safer discharge of low-risk patients and tailored social prescribing by creating a pathway with clinicians specialising in safe discharging and with excellent knowledge of community links and wellbeing offers.
The main responsibilities of this role will include meeting with patients for social prescribing, completing final routine outcome measures, writing discharge letters, assessing risk before discharge from community CAMHS with the support of the MDT.
Kensington and Chelsea CAMHS is well established multi-disciplinary team. We aim to provide an accessible, flexible and responsive local service for patients and professionals working with children in the area. We use a diverse range of therapeutic interventions, including Cognitive Behavioural Therapy, systemic and family therapies, psychotherapy, psychoeducation and therapy groups and multi-family work. Kensington and Chelsea CAMHS consists of five main sub-teams: Emotional Disorders Team; Neurodevelopmental Team; Enhanced Treatment Team; Duty team; and under 5s Team.
