We are looking for a highly motivated, well organised, enthusiastic individual to work flexibly as Discharge Facilitator within Surgery.

You will be responsible for improving the patient’s experience and supporting effective, timely and safe discharges by means of providing practical support to the multidisciplinary team and nurse in charge in accordance with departmental and Trust policies and procedures.

You will be expected to have excellent communication and organisational skills and the ability to work autonomously as part of a multi-disciplinary team.

Requirements
• Healthcare/rehabilitation/ therapy assistant or adult social care experience.
• Excellent communication skills.
• Adaptable and flexible approach.

The post holder will be required to undertake practical support for discharge planning within surgical wards to reduce length of stay, delayed transfers of care and increase the number of timely discharges.

In addition the post holder will need to ensure the discharge planning process is responsive to the needs of patients and their carers by means of providing supportive discharge care.

Other responsibilities include being visible at ward level to ensure that discharge planning is proactive and timely and that patients are prepared and supported for their expected discharge day and time.

Daily chasing of the active request for package of care, clerking of new patient and weekly/ daily discussing with Multi-professional team regarding planning for safe discharge.
