An exciting opportunity has arisen for a motivated and proactive individual to join the multidisciplinary Palliative Care team based at Pembridge Hospice.
Our service supports people living with life-limiting illness to live as well as possible, focusing on rehabilitative palliative care that promotes independence, comfort, and quality of life. The Therapy Assistant role supports the Occupational Therapist and Physiotherapist in delivering person-centred rehabilitative palliative care to patients. Interventions are delivered face-to-face and online, across outpatient appointments, group sessions, and home visits.
This post includes a mix of clinical and administrative responsibilities and offers a comprehensive induction and training programme to support skill development and competence in the role.
The service is based at St Charles Centre for Health and Wellbeing, a short walk from Ladbroke Grove station, and provides care to patients across Brent, Kensington and Chelsea, and Hammersmith and Fulham.
• Support and assist the Occupational Therapist and Physiotherapistin delivering rehabilitative palliative care to patients in a variety of settings, including outpatient clinics, group sessions, home visits, and virtual appointments.
• Carry out delegated clinical tasksin line with care plans, competencies, and service protocols, with flexibility for reasonable adjustments when required.
• Assist with preparation, delivery, and follow-upof therapy interventions, including equipment provision and basic exercise programmes as appropriate to the patient’s needs.
• Undertake administrative dutiesto support the therapy service, including record keeping, appointment coordination, and data collection.
• Communicate effectivelywith patients, families, carers, and members of the multidisciplinary team, respecting diverse communication needs and ensuring coordinated care.
