Support Worker - Grade D

Description
As a Support Worker - Grade D, you will be an integral part of our dedicated team committed to enhancing the quality of life for individuals in need of support. This role is pivotal in providing compassionate and personalized assistance to clients with varying support needs, including those with physical disabilities, mental health challenges, and learning difficulties. Your day-to-day activities will involve working closely with clients to encourage their independence while providing the necessary support to ensure their safety and well-being. You will be responsible for facilitating social interaction, promoting life skills, and supporting clients in daily tasks to help them achieve their individual goals. Through effective communication and empathy, you will build strong relationships with clients and their families, fostering a trusting environment that promotes holistic development. Responsibilities
Assist clients with daily living activities such as personal hygiene, meal preparation, and household tasks.
Support clients in developing life skills and encourage independence.
Administer medication and monitor health conditions as directed by healthcare professionals.
Provide emotional and social support, facilitating engagement in community and recreational activities.
Maintain accurate records and documentation of client progress and care provided.
Collaborate with multidisciplinary teams to develop and implement individual care plans.
Participate in training and development activities to enhance professional skills and knowledge.
Requirements
Minimum of a high school diploma or equivalent; relevant certifications in social care are preferred.
Proven experience in a support role or similar environment is advantageous.
Strong interpersonal and communication skills, with the ability to build rapport with clients and their families.
Empathy and a genuine interest in improving the lives of individuals with various needs.
Ability to work independently as well as part of a team, demonstrating strong teamwork skills.
Flexibility in working hours, including evenings and weekends as required.
Understanding of confidentiality and ethical considerations in care provision.
