Crynodeb
1. To provide specialist assessment and intervention within the Behavioural and Psychological Symptoms of Dementia (BPSD) pathway in particular working with the service user, family, care home staff to manage these behaviours. This can be either a nursing or OT approach under a psychological formulation model of care. 2. Support the Guided Care Matron with managing referrals for the BPSD pathway. 3. To work alongside the Senior Clinical Psychologist and Guided Care Matron to manage a caseload for patients under the BPSD pathway; under a psychological formula based model of care. 4. To support in the patient within their residence or Care Home to remain there, maintaining safety and reduce the need for acute mental health hospital admissions. 5. To guide and instruct the OTA/ Dementia Care Assistant in supporting and signposting patients so they can engage in meaningful activities that support BPSD. 6. To act as a resource for services in the community that support health and wellbeing to people living with Dementia. 7. To support the Guided Care matron and MH Care Home support team in the Care Home Forums. 8. To support in the monitoring, maintenance and promotion of the physical health and wellbeing of patients with mental health concerns.
