The Move-On & Resettlement Officer plays a crucial role in supporting individuals transitioning to new phases in their lives, ensuring they have the necessary resources and guidance to successfully navigate the resettlement process. Responsibilities
Assess the needs of individuals in transition and develop tailored resettlement plans.
Coordinate with local agencies and organizations to facilitate access to housing, employment, and healthcare services.
Provide individualized support and guidance throughout the resettlement process, including attending appointments and filling out necessary paperwork.
Conduct outreach to educate individuals about their rights and available resources in the community.
Monitor and evaluate the progress of resettled individuals, making adjustments to plans as necessary to ensure successful outcomes.
Organize and facilitate workshops and informational sessions on topics relevant to resettlement and integration.
Maintain accurate records of services provided and outcomes achieved, ensuring confidentiality and compliance with relevant regulations.
Requirements
Bachelor's degree in social work, psychology, community development, or a related field.
Experience in case management, social work, or a similar role focused on resettlement or transition support.
Strong interpersonal and communication skills, with the ability to build rapport with diverse populations.
Knowledge of local community resources, agencies, and support services.
Proficiency in using case management software and Microsoft Office Suite.
Ability to work independently and as part of a collaborative team in a dynamic environment.
Cultural sensitivity and awareness, with a commitment to social justice and advocacy.
Hours Per Week: 37.00
