Summary
Key Responsibilities Clinical Care Provide comprehensive mental health assessments, formulation and care planning for patients presenting in primary care Deliver evidence-based interventions for common mental health conditions (e.g. anxiety, depression, PTSD, severe mental illness) Undertake risk assessments, crisis management and safeguarding interventions Manage a defined caseload and work autonomously with supervision as required Liaise with GPs, multidisciplinary teams, and secondary mental health services to ensure coordinated care Support long-term condition management for patients with serious mental illness Leadership and Service Development Provide clinical supervision, mentorship and support to junior mental health staff and trainees Contribute to service development, quality improvement initiatives and PCN mental health pathways Participate in audits, policy development and clinical governance activities Act as a mental health resource and advisor within the PCN Education and Health Promotion Deliver psychoeducation to patients, carers and families Support mental health promotion, prevention and early intervention initiatives across the PCN population
