CLCH Hertfordshire Adult Community Services- CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

An exciting opportunity has arisen within our in-patient Neurological Rehabilitation service. We are looking for a band 3 Clinical Support Worker to join the Nursing team at Holywell Neurological Rehabilitation Unit, Langley House.

This is a service for the rehabilitation of adults post stroke and other range of Neurological illnesses. We are looking for a motivated individual to join the team, who has the passion and skills for supporting patients and working along with the nursing team in the delivery of a high standard of rehabilitation care. You will be working with a multidisciplinary team that includes Nurses, Physiotherapists, Occupational Therapists, Dietitian, Clinical Psychologist, Speech and Language therapists, Social Workers and other Rehabilitation Assistants.

To work with all members of the multi-disciplinary team (MDT) in planning and facilitating the patient’s progression through the treatment program, within the goals and protocols set out by the qualified staff.

To assist nurses in completing paperwork, referrals and other appropriate forms that may be required during rehabilitation.

To contribute to the on-going assessment of a patient by completing the appropriate paper work of allocated patients, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning.

To communicate effectively with patients, relatives and colleagues at all times in a manner consistent with the knowledge and skills of HCA

To maintain at all times a kind, caring and professional attitude towards patients, relatives and colleagues within the clinical setting, taking into consideration dignity, privacy, confidentiality, cultural and religious needs.

To undertake and report back on delegated clinical activities that has been assigned, eg, vital signs, blood glucose monitoring

To take responsibility for delegated caseload once appropriate level of competency achieved and report outcomes appropriately, including reporting urgent and important information promptly to the appropriate person.
