Summary
Trust Resident Doctor
The Trust is advertising for Trust Junior Doctors to work on our in-patient wards.
The post offers the applicant an exciting opportunity to gain experience in Care of the Elderly.
The Department of Medicine for Older People has a friendly and informal atmosphere where the junior doctors feel well supported and achieve their educational targets. We aspire to high standards of medical and nursing care, and routinely assess aspects of this with regular clinical auditing. Mentoring will be provided by the Trust’s mentor scheme for any new consultants.
Regular involvement in clinical governance including morbidity and mortality meetings and management meetings, service development, audit and teaching will be expected. There is also an active educational programme, and all members of the care of Older People’s team will be encouraged to attend and contribute.
Take a good social history. Document whether they live in a house, flat or in a residential/nursing home. Also document their pre-admission level of functioni.e. activities of daily living (washing, dressing, feeding, shopping, cooking) as well as reliance on walking aids and support services. This information helps with setting rehabilitation goals.
As most elderly patients have multiple medical problems it will be helpful to draw up a problem list highlighting active medical problems and any past medical history. If the patient has been transferred from another ward it is helpful to summarize investigations and results and to list investigations that are pending.
Talking to relatives can give us valuable information about a patient's social set up and about problems that have been an issue at home / barriers to discharge. It is best to gain this information early on. Many of our patients have cognitive impairment and may not be able to give a reliable history. Conversely, they may not be able to pass information onto their relatives/NOK and so liaising with relatives keeps them in the loop. We aim to update families twice a week. Bear in mind issues of confidentiality.

Acute Frailty Service
There is an established Acute Frailty Service which comprises a Multidisciplinary Team based in ED, CDU and Acute Care Unit giving rapid and responsive Comprehensive Geriatric Assessment to older patients with frailty at the front door. This service creates individualised plans for patients with the aim to avoid admission if possible or reduce the length of stay if admission is required. There is opportunity to be involved with the Acute Frailty Service which involves leading the Frailty MDT (currently 2 Frailty Practitioners, one frailty nurse, one frailty OT, a frailty pharmacist and an SHO) to assess appropriate patients presenting ED and CDU and EACU.
OPRAC
The Older Persons Rapid Assessment Clinic (OPRAC) is a well-established service providing Comprehensive Geriatric Assessment in an outpatient setting. We have a mix of virtual and face to face appointments.
OPRAC is now combined with Ealing Acute Care Unit so that older patients with frailty, who have historically been excluded from ambulatory pathways can have access to same day emergency care (SDEC) – avoiding unnecessary and potentially harmful admissions. There is an opportunity for the post holder to create a rapid access clinic of their own interest e.g. heart failure clinic.
Sub-specialty interests
The Trust’s priority is to develop interface work such as Orthogeriatrics and Surgical liaison in parallel to the traditional ward based CGA on the elderly care wards. Development of any special interests is encouraged and will be actively supported– e.g., Dementia, Continence, Falls, Movement Disorders, Surgical liaison including Orthogeriatrics, Cardiology liaison and Community including potential involvement in the Ealing Frailty Virtual Ward. Time within the job plan to explore and develop services related to these can be negotiated.
