As a care coordinator, you will play an integral role in the quality of the company’s service delivery. Although full training will be provided, experience in bespoke IT systems Access People Planner would be an advantage
You will need to assist clients in the absence of a carer, that is to undertake care calls in extreme emergencies. You will be expected to take the on-call phone weekly. You may need to conduct initial assessment, create care plans and risk assessments for new customers, review existing care plans, support customers and their families, and support and train care workers.
You will also be required to shadow new carers into their role, conduct spot checks, supervision and staff appraisals. Furthermore, you will be responsible for updating staff matrix, ensuring compliance is met.
You will also be expected to deal with staff request for annual leave. No annual leave should be assigned to carers until approved by the registered manager.
You will be responsible for assigning the most appropriate care workers to our customers. Following the creation of a customer care plan, you will make recommendations to the operational manager, taking into account any cultural or specific requests made by the customer.
When you visit someone who is considering our service, you will need to be able to quickly create a good rapport, reassuring those who are new to receiving home care, and carefully listening to their personal needs.
You will have a good working knowledge of person-centred care and be able to create strong working relationships with other care professionals including social workers, GP’s, Pharmacist, and District nurses who also work around our customers.
The company is looking for a team player who can demonstrate hunger, drive, and a strong eye for detail. The role also includes performing many of the associated admin duties like managing care/client files and producing system-generated invoices and compliance documents.
