Dorset Healthcare are looking for Community Therapy Assistant Practitioner to join our supportive Purbeck ICRT based here in Wareham.
The multi-disciplinary team work with adult patients in their own homes or care homes to promote independence and improve quality of life by supporting them to remain at home. Therefore being a car driver is essential for this role. You will be joining a motivated, caring, inclusive and compassionate team. You will work with a team of Nurses and Therapists to set goals and assist patients to regain their independence.
The role of Therapy Assistant Practitioner may also be required to work with patients on the ward at Swanage Hospital to support their rehab goals and help them to return home. We offer opportunities for further learning and development, including the chance to apply for Nursing, Physiotherapy and Occupational Therapy Apprenticeships.
The Therapy Assistant Practitioner role will be required to carry out holistic assessments, formulate treatment plans and monitor progress against set goals, under the indirect supervision of a registered therapist.
This role is a full-time post of 37.5 hours per week. Hours covered by the service are 08:00-20:00 7 days per week with weekend enhanced pay!

The successful candidate will gain knowledge and experience alongside a range of colleagues, within a mutually supportive team. They will benefit from regular in-service training, and support through supervision and annual Professional Development Reviews, and contribute to the training and supervision of rehabilitation assistants.
You will be required to carry a clinical caseload of patients with orthopaedic health conditions including falls and trauma, ensuring that patients benefit from high quality, evidence based interventions, which you will deliver and record according to professional standards and legal requirements.
This is an ideal opportunity if you have clinical interest in community therapy, and a strong motivation to develop skills and expertise and provide patient centred, evidenced based therapy.
You will require a car for use at work, to enable you to work autonomously in the community to visit patients, whilst adhering to local and Trust guidance on lone working.
