THE POPLARS MEDICAL CENTRE JOB DESCRIPTION Job Title: Practice Pharmacy Technician Location: Poplars Medical Centre, Swinton Position: Full-time / Part-time, Permanent Salary: Depending on experience Agenda for Change Band 5 (£31,049-£37,796) Hours of Operation: 8.00am 6.30pm, Monday to Friday Accountable to: Practice Partners / Practice Manager Line Managed by: GP Lead Practice Pharmacist JOB SUMMARY The post holder will work as part of the practice pharmacy team, supporting medicines optimisation, repeat prescribing processes and medicines-related administration. The role is essential to ensuring safe prescribing systems, efficient workflows and high-quality patient care. The Pharmacy Technician will work under the clinical supervision and line management of the GP Lead Practice Pharmacist. KEY RESPONSIBILITIES Undertake medicines reconciliation from hospital discharge summaries and clinical correspondence, ensuring accurate and timely updating of patient records. Support medicines management activities, including prescription screening, processing, and optimisation. Assist with and oversee the repeat dispensing process to ensure safe, efficient continuity of care. Respond to and communicate effectively with patients regarding medication-related queries. Liaise with community pharmacies to support stock management and mitigate medication shortages. Coordinate medication synchronisation in line with patients regular repeat prescription cycles. Oversee medication recall and monitoring processes, ensuring timely action and compliance with required reviews (e.g. blood pressure checks, blood tests, annual medication reviews). Support Direct Oral Anticoagulant (DOAC) monitoring in line with local and national guidance. Participate in prescribing and medicines optimisation audits, including those linked to CQC, QOF, and local incentive schemes, as directed by the Prescribing Team or ICB (training provided where required). Support high-risk drug monitoring to promote patient safety. Review and act on existing and new Shared Care Protocols in accordance with agreed guidance. Action medicines safety alerts and recalls issued by CAS, MHRA, ICB, or other relevant bodies. Create and run clinical system reports to identify, stratify, and prioritise patients for medicines review. DIGITAL SYSTEMS AND WORKFLOWS Use EMIS, Ardens, AccuRx and EPS to support medicines-related workflows. Support improvements to prescribing processes to reduce administrative burden. Accurately code and record medicines-related activity. Support data collection for audits and quality improvement. TEAMWORK AND COMMUNICATION Work closely with the GP Lead Practice Pharmacist and wider MDT. Communicate effectively with patients regarding medicines queries within scope. Support induction and training of staff in medicines-related processes. Contribute to a supportive and collaborative team culture. CLINICAL GOVERNANCE Work in accordance with CQC, NHS and practice governance requirements. Adhere to medicines management SOPs and safety protocols. Participate in audits, significant event reviews and learning activities. Maintain confidentiality, data protection and safeguarding standards.
