Crynodeb
The Cherwell Hospital seeks a Pre-Assessment Nurse to deliver expert nursing care and assessment for pre-assessment patients. The role involves facilitating evidence-based, patient-centered care for a defined caseload. Work independently and with a team of nurses and healthcare assistants to coordinate patients in their pre-assessment pathway, managing care prior to admission. Support all staff in the Outpatient department, including nursing students and visiting professionals. Act as a liaison with the Multi-disciplinary team, contributing to Anesthetic and Complex MDT meetings, escalating complex cases and planning care. Use advanced communication skills to guide patients and staff on admission management and its impact on lives. Communicate complex, sensitive information to patients, overcoming barriers to understanding. Establish partnerships with patients/carers, even in distressing situations. Advocate for patients, ensuring informed consent, privacy, and dignity (NMC). Reflect on own performance through supervision/action learning.
