We are seeking an experienced Advanced Clinical Practitioner (ACP) to join our clinical team. You will work autonomously to: Assess, diagnose, and manage patients with acute, chronic, and complex conditions Prescribe and deliver treatment plans, therapies, and lifestyle advice Provide chronic disease management and health promotion Support safeguarding, clinical emergencies, and multidisciplinary care Contribute to clinical governance, audits, and service improvement
