An opportunity has arisen, for a 6 month fixed term post, for an enthusiastic Nurse to be the Nursing Team Leader at the ARC, Assessment and Rehabilitation Centre, in Blackpool. The ARC provides an Intermediate Care Service to support patients to optimise their independence and quality of life and is proud to enable patients to realise their potential through the integrated team across health and social care which includes Physiotherapy, Occupational Therapy, Nursing, Social Workers and Rehabilitation and AHP support workers.
There is a dedicated team of nurses who are supported holistically where in-service training and supervision is provided.
We are looking for a team leader with an excellent ability to communicate at the appropriate level, verbally and in writing, with colleagues, senior clinicians, carers and patients is essential. You must be both methodical and organised with skills in working under pressure, as part of a multi- professional and agency team and at times working in isolation.
The successful candidate must be able to undertake cross boundary working and ensure the caseload is managed effectively.
The post holder will be responsible for managing the deployment and supervision of the nursing staff within the integrated team at the ARC, across health and social care.
Responsibility for the leadership and management of the day-to-day delivery of service including managing issues relating to HR, performance management and delivery of service KPIs and quality measures, working collaboratively across health and social care.
Working under the direction of the Deputy Head of Locality and with colleagues and key stakeholders to ensure the delivery of a coordinated, multidisciplinary and multi-agency service. This includes planning, delivering, evaluation and development of the services for which they are responsible.
Monitoring and analysing performance data against agreed targets for the team.
Developing and agreeing action plans to address performance issues.

Leadership and Professional Responsibilities
• Provide leadership to the ARC ensuring an engaged and empowered workforce
• Maintain an active status on the Nursing and Midwifery Council (NMC), acting in accordance with Code of Professional Conduct, maintaining own competency to practice.
• Be professionally and legally accountable for all aspects of own work, including the management of patient caseload, as appropriate, in accordance with the relevant standards of practice and Trust clinical practice guidelines.
• Keep up to date with practice development or professional issues, including attending all statutory and mandatory training as stipulated by the organisation.
• Ensure staff, for which the post holder has responsibility, maintain practice development and professional standards.
• Contribute to the improvement and maintenance of good working relationships and effective joint working between other services and agencies.
• Take action in line with trust policy on any professional or practice development issues within the team and advise the Deputy Head of Locality where appropriate.
• Be responsible for the coordination and provision of clinical placements within a defined clinical area.
• Ensure the team provide consistently high standards of care to patients referred to the service, which is evidence based, and in accordance with national/local clinical practice guidelines.
Clinical Responsibilities
• Manage a defined clinical caseload effectively and efficiently with regard to clinical priorities and the use of time, if appropriate
• Undertake individual health needs assessments of patients/ clients, formulate a clinical opinion, discuss care options, provide advice, training and instruction to patients/ clients, relatives and carers, working collaboratively with the MDT at the ARC, to enable understanding of proposals/ treatment plans and make recommendations and agree a goal orientated programme of care.
• Gain informed consent for treatment and have the ability to work with patients/ clients who lack the capacity to give consent. This will involve acting as a resource for staff on difficulties regarding consent issues.
• Give advice and make decisions on the prescription of medical devices to be used to support patient/client care.
• Provide evidence based advice, guidance and support on complex cases to staff within the team.
• Prepare and present clinical reports, if/ as required, at relevant social services, educational and medical case conferences
