Crynodeb
To provide high quality assessments and interventions for children and young people within the CIPPRD both independently and as part of a multi-disciplinary team. To comprehensively assess, manage and monitor a range of physical and mental health difficulties. Providing integrated physical and mental health care alongside the multidisciplinary team To provide care-coordination and assist with the discharge process of patients. Including liaison with local services for safe handover and preparing the necessary documentation. To contribute to the effective working of the CIPPRD. This will include providing written documents and reports in a timely manner as needed as part of patient care. To work in accordance with clinical guidelines and the policies and procedures of the service, seeking help from clinical/professional supervisor or manager as needed.
