Core Responsibilities and Duties of the Advanced Clinical Practitioner (ACP): Conduct comprehensive assessments of patients, including physical examinations and health histories. Manage patients with a wide range of acute and chronic conditions, providing expert clinical care. Develop, implement, and evaluate individualised care and treatment plans based on patient needs and evidence-based practice. Prescribe medications and initiate other appropriate clinical interventions within scope of practice. Work in partnership with individuals, families, and carers using a variety of assessment and communication methods. Collaborate with multidisciplinary teams to ensure coordinated, person-centred care. Demonstrate effective communication skills to support shared decision-making and positive health outcomes. Perform diagnostic procedures, interpret results, and use clinical reasoning to guide treatment. Maintain accurate, up-to-date clinical records in line with legislation, using SNOMED CT coding effectively. Provide chronic disease clinics and refer patients to secondary or specialist care as needed. Identify, manage, and support patients at risk of developing long-term conditions to prevent deterioration. Deliver opportunistic health promotion and patient education where appropriate. Support the practice in safeguarding matters and respond to clinical emergencies. Act as a professional role model, demonstrating accountability, emotional intelligence, and critical reflection. Actively engage in peer review, feedback processes, and service improvement initiatives. Critically assess and address personal learning needs, engaging in continuous professional development across clinical practice, leadership and management, education, and research. Contribute to and help implement quality improvement projects, governance systems, and practice performance targets (e.g., QOF). Chaperone patients when necessary and follow local and national policies regarding substance misuse and addiction. Stay current with clinical developments through ongoing education and professional engagement. Understand and work within relevant legislation, policy frameworks, and the limits of professional competence.
