To identify potential clients for the pathfinder partnership via waiting listscreening and engagement with ISVAs To present potential clients at consultation via the confidential consultation processalso liaison with referrers and other service providers to ensure clients are treated in the most appropriate service for their needs. To provide a thorough bio-psycho-social assessment of the client (aged 16+), thiswill include a risk assessment and a care plan. To deliver individualised interventions to the client based on individual care plan Liaison with all members of the pathfinder partnership, HSP clinicians, Secondary Care Mental Health Services, Voluntary /Statutory agencies across Greater Manchester To manage own caseload by delivering brief interventions to the client, and participating in line management to plan endings and allocation of new clients.
