Crynodeb
The Care Coordinator will provide administrative, organisational, and patient focused support to help improve care delivery and patient experience. The post holder will work closely with GPs, nurses, healthcare assistants, and the wider practice team to support patients, particularly those with long term conditions or additional support needs. The role involves coordinating care, assisting patients in navigating healthcare services, supporting clinics, and contributing to the smooth running of the practice.
