Job responsibilities: 1. The post holder will, under appropriate supervision provide first point of contact for patients presenting with undifferentiated, undiagnosed problems, utilising history taking, physical examination, problem solving and clinical decision-making skills to establish a working diagnosis and management plan working in conjunction with patients. 2. The post holder will undertake both morning and afternoon clinics as well as home visits and administrative work. 3. Participate in treatment and preventative healthcare. 4. Provide direct clinical care to patients using established clinical guidelines. 5. Consult and refer patients to other healthcare professionals. 6. Provide one to one consultation to patients, both in General Practice and patients homes, taking medical histories, perform physical examinations, analyse the consultation, diagnose and explain the medical problems during the consultation. 7. Recommend and explain appropriate diagnostic tests and treatment. 8. Provide patient education in preventative healthcare. 9. Request laboratory tests as required and agreed under supervising GP. 10. Update patient records in accordance with practice policies and procedures. 11. Contribute to the practice systems to manage common chronic medical conditions. 12. Ensure observance and adherence to the Health and Safety at Work Act 1974. 13. Work to the agreed standards, policies and procedures and protocols of the Practice and be fully conversant with emergency procedures. 14. Carry out other duties as directed by the GP Partners/Practice Manager which are consistent with the duties and responsibilities of the post.
