Description
We are seeking a dedicated and compassionate Adults Senior Social Worker specializing in Hospital Discharge to join our dynamic team. This role is vital to ensure that patients transitioning from hospital care to their homes or other care settings do so with the necessary support and resources. As an Adults Senior Social Worker, you will take an active role in assessing patients’ needs, collaborating with multidisciplinary teams, and developing effective discharge plans that prioritize patient well-being and safety. Your expertise will guide families through complex emotional and practical considerations, ensuring they receive the right community resources and follow-up care. This position requires a strong commitment to improving patient outcomes through effective communication and problem-solving skills. You will also be responsible for supervising junior staff and student placements, providing mentorship and enhancing the overall service delivery. Responsibilities
Conduct comprehensive assessments of patients' social needs before discharge.
Collaborate effectively with healthcare professionals to develop and implement discharge plans.
Provide emotional support and guidance to patients and their families during the discharge process.
Ensure patients have access to community resources, services, and follow-up care after discharge.
Supervise and mentor junior social workers and students in their professional development.
Document assessments, interventions, and outcomes in compliance with legal and organizational standards.
Evaluate the effectiveness of discharge plans and make necessary adjustments to improve patient outcomes.
Requirements
Valid social work qualification and professional registration with the relevant body.
Significant experience in social work, particularly in hospital settings and discharge planning.
Strong understanding of social care legislation, policies, and best practices related to adult care.
Excellent communication and interpersonal skills to engage with patients, families, and multidisciplinary teams.
Proven ability to assess complex situations and provide effective solutions.
Experience in mentoring and supervising less experienced staff or students.
Demonstrated commitment to continuous professional development and staying updated with relevant social work practices.
