Crynodeb
Working within a multi-disciplinary team of health and social care professionals, providing support for clinical care to patients closer to home as part of the patient pathway from the acute sector through to discharge. To provide support, assessment and treatment for patients within their own homes providing appropriate aspects of exercise, activity and treatment programmes which may be prescribed by a clinician, with the aim of rehabilitation and optimising potential. To carry out non-clinical activities which facilitate the efficient running of the team (e.g. clerical support).
