Summary
Key ResponsibilitiesQOF and Data Management Maintain accurate, complete, and up-to-date patient records using the SystmOne clinical system. Input, review, validate, and code QOF-related clinical data in line with national and contractual requirements. Generate, analyse, and interpret QOF and performance reports to identify gaps and areas for improvement. Monitor QOF indicators proactively and take appropriate action to support achievement of targets. Ensure all patients eligible for QOF reviews are identified, recalled, and followed up annually. Recall and Monitoring Systems Manage and oversee patient recall systems for chronic disease management, QOF, and non-QOF recalls (e.g. DOAC monitoring). Add and maintain recalls triggered by secondary care instructions. Use MJOG SMS and other communication methods to invite patients for reviews and track responses. Monitor uptake and attendance, escalating concerns where required. Clinical Correspondence and Results Download and process clinical correspondence via GP Access and practice email inboxes. Attach letters to patient records, apply appropriate clinical coding, and forward to the relevant clinician. Distribute incoming pathology results in accordance with practice protocols. Reporting, Audit, and Compliance Prepare and submit QOF progress and performance reports for internal review and external audit purposes. Support clinical audits, inspections, and contractual reviews as required. Keep administrative Standard Operating Procedures (SOPs) accurate and up to date. Remain up to date with changes to QOF, contractual requirements, and NHS guidance, ensuring the practice remains compliant. Practice Manager and Team Support Work closely with the Practice Manager to develop and implement strategies to maximise QOF achievement. Regularly review performance data with the Practice Manager and highlight risks or underperformance. Provide administrative and data support to clinicians and the wider practice team. Support staff understanding of QOF processes and clinical systems where appropriate.
