Nurse Seekers are proud to be recruiting a Community Complex Care Nurse on behalf of a specialist complex care provider. This is a community-based role supporting vulnerable adults with highly complex health needs within their own homes.

This is a rewarding opportunity for a skilled and compassionate Registered Nurse who is confident working autonomously while delivering exceptional, person-centred care.

The Role

As a Community Complex Care Nurse, you will provide specialist nursing support to individuals with complex clinical needs, ensuring the highest standards of care, safety, and clinical governance are maintained at all times.

You will work in line with the NMC Code of Professional Conduct, maintaining professional accountability for your practice while supporting patients, families, and multidisciplinary teams.

Key Responsibilities

• Assess, plan, implement, and evaluate nursing care for adults and children with complex health needs
• Deliver specialist clinical care including:

• Long-term ventilation
• Tracheostomy care
• Oxygen therapy
• Suction
• Enteral feeding

• Provide skilled, evidence-based nursing care in community and home settings
• Safeguard children and adults at risk and follow safeguarding policies at all times
• Act as a clinical lead within allocated care packages when required
• Support, train, mentor, and supervise healthcare support workers
• Undertake competency assessments, supervisions, and appraisals
• Provide education and guidance to patients, families, and carers
• Carry out home risk assessments to ensure patient and staff safety
• Maintain accurate and contemporaneous paper and electronic records
• Liaise with:

• Families and carers
• Education professionals
• Multi-disciplinary and multi-agency teams

• Participate in audits, clinical governance, and service development
• Maintain supplies and equipment required for individual care packages
• Travel within the community to visit clients as required

Person Specification

Essential Requirements

• Registered Nurse qualification (Degree level or above)
• Active NMC registration
• Commitment to the NMC Code of Conduct and professional standards
• Experience supporting adults and/or children with complex health needs
• Strong clinical knowledge and evidence-based practice
• Ability to work independently and manage clinical decision-making
• Excellent written and verbal communication skills
• Ability to work flexibly within a community-based service
• Commitment to safeguarding, infection control, and quality assurance
• Willingness to travel within the TN11 area

Desirable Experience & Skills

• Tracheostomy care, ventilation, enteral feeding, catheterisation
• Acting as a lead nurse within care packages
• Experience of regulatory inspections
• Knowledge of clinical audit and research-based practice
• Experience working within regulated care environments
