This post is not open to trainees therefore applicants must have completed the Diploma in Health and Social Care Level 2 or equivalent. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role). Detailed job description and main responsibilities To work within the policies, protocols and clinical procedures of the Trust and to promote competent working practice within a supportive multi-disciplinary team. To liaise with Mental Health Nurses, Occupational Therapist and Physiotherapy staff within the team and carry out assigned duties as a member of the multi- disciplinary mental health team. To be prepared to work on own initiative as necessary Promote equality for all patients and understand their health needs. Demonstrate an awareness of service user and carer perspectives on the provision of treatment and continuing care. Ensure accurate record keeping in accordance with Trust policy. Participate regularly in clinical supervision sessions. Liaise with representatives from associated voluntary and statutory organisations e.g. social care and facilitate good communication processes. Attend in-service training and other study / refresher days to meet individual and service needs. Participate in performance reviews. Maintains safety, privacy and dignity of all patients in the delivery of patient centred care, recognising and respecting differences including spiritual and cultural beliefs. Responsible for recognising the signs of patient harm, abuse or neglect, including poor clinical practice, identifying issues regarding the safeguarding who may be at risk and reporting all such concerns in accordance with Trust Please refer to the attached job description and person specification for further detailed information to ensure that you meet the role criteria before applying.
