Job Summary You will be: a clinical professional who has developed their skills and theoretical knowledge to a high standard. an experienced and educated member of the care team who are able to diagnose and treat health care needs or refer patients to an appropriate specialist if needed. Trusted and empowered to make high-level clinical decisions and will often have your own caseload. As an ATC team member, you will be expected to perform many of the tasks and take on a similar level of responsibility to trainee medical staff who have a formal regulator in the GMC. These duties may be performed with direct, indirect, or distant supervision by a GP Lead or similar, depending on the task and situation. You will have excellent communication skills, a compassionate and friendly nature who can operate in a busy environment with a range of people and professions, as you will be working our clinical and administration teams. Job Responsibilities CRITICAL CARE Undertaking comprehensive clinical assessments of patients Requesting and performing tests to diagnose a patient's condition. Undertaking certain invasive interventions (such as central venous access) Coordinating the transfer of patients within Practice (e.g. to hospital, community hospital or other) Managing episodes of patient care Leading and supporting within a team of different healthcare professionals and staff. Using different sources of information to assess and manage the care of a critically ill patient. Undertaking clinical audits Work as part of the Acute Care Team, alongside other health professionals, and provide support and training for others as required. Carry out home visits as needed, working to safe lone worker considerations. Triage and assess patients from information provided via digital triage model, or telephone assessment and arrange treatment of follow-up as required. Assist the Duty GP in the management of the acute daily work. PATIENT ASSESSMENT Evaluating and/or collecting information from physiological assessments, patient history, and diagnostic data to identify relevant problems. Advanced levels of patient assessment Diagnostics and decision-making Advanced patient interventions Advanced non-medical prescribing Using clinical decision making to initiate and adjust appropriate management/treatments according to agreed protocols and clinical judgement. Evaluating patients progress against treatment plans through interpretation of information from test results and clinical findings, invasive and non-invasive monitoring systems PATIENT SAFETY AND QUALITY IMPROVEMENT Participate and lead in the collection and evaluation of clinical audit data. Contribute to and lead in relevant directorate audit and research programmes as identified or requested. Lead on relevant departmental audit programs Utilise evidence-based knowledge within practice development. Support and promote an integrated approach to quality, seeking always to improve patient care. Contribute to the clinical governance agenda within the Practice as required. RESEARCH, EDUCATION AND DEVELOPMENT To provide clinical and professional support and advice to all staff. Develop and deliver specific and relevant teaching programs for all disciplines. Support, supervise and assess clinical competencies of nursing staff and other members of the multidisciplinary team. Maintain accurate records and provide necessary data for statistical analysis. Implement research findings appropriate to practice. Initiate new and support existing research projects. To assess, monitor and evaluate the role of the Advanced Practitioner out of hours. To act as a role model ensuring own actions promote quality and identify and manage any risks. To raise quality issues and report related risks within the Practice and/or local healthcare network to address poor standards and incidents. To inform and influence clinical governance issues. To keep up to date and act consistently with quality standards and guidelines within their own clinical area and associated areas To undertake the measurement and evaluation of your work and current practices though the use of evidence-based practice projects, audit and outcome measures, and lead others in doing so. Make recommendations for and lead change. CLINICAL AND MANAGERIAL LEADERSHIP Provides effective leadership and acts as a positive role model. Delegate appropriately and encourage staff to develop and advance their skills. Ensure the clinical team are kept informed of the progress of the role and provide opportunities for suggestions and comments to improve care delivery. Communicate well with staff and use different communication approaches to ensure team are conversant with relevant information. Be pro-active in the management of change and can support staff through times of change.
