See Job description for details Internal Relationships:To communicate effectively, with Patients, carers and other staff imparting complex and sensitive informationand unwelcome news. Senior physiotherapy staff to maintain a coordinated approach to service delivery and development. With qualified physiotherapy staff, physiotherapy assistants and students on clinical placement to support team working on a day to day basis With medical, nursing, paramedical, and other hospital staff to promote a coordinated approach to patient care and multi-disciplinary team working in the preparation and implementation of treatment/care policies, procedure and carepathways. Senior Managers Other Trust departments External Relationships With all disciplines in the community, particularly other physiotherapists, to provide continuity of care across the Acute Health Services Unit, Social Services, the West Kent Primary Care Trust, and Carers. To contribute to and execute the Patients Care Plan through effectivecommunication with the patient, family and carers.
