Crynodeb
The focus of the post holder will be on providing evidence–based interventions for the urgent and emergency care pathway in general hospitals with the aim of reducing length of stay and frequent attendances to hospital. You will be working across working age and older adult services. You will be carrying out occupational therapy and psychosocial assessments of people referred to the Liaison service from a variety of sources within the general hospital, including the emergency department (ED) and medical wards. To work with people presenting with a range of psychosocial needs arising in the context of physical illness, as well as people presenting to the hospital following self- harm or in a mental health crisis.
Key Responsibilities:
• To provide rapid psychiatric assessment and safe discharge planning in the ED.
• To lead the care for a specified group of service users such as frequent ED attendees
• To agree on a collaborative plan of care with people who use our services and their carers
where appropriate and arrange appropriate after-care
• To support physical acute services in managing the mental health needs of patients in the
acute setting and set robust plans to support their discharge
• To ensure that the relatives, carers and friends are actively encouraged to participate in
care, provided with information, help and support where necessary
• To mentor, supervise and teach qualified and unqualified staff including students on working
with patients who experience mental health difficulties and cognitive impairment
• To facilitate the teaching and training of mental health topics in the general hospital

• To assist in the development and maintenance of good working relationships with general
hospital staff, local mental health services, primary care and non - statutory organisations
• To work flexibly as the liaison service develops to ensure the service level agreement is met
and the changing needs of people who use the service in the hospital are met
Working across two busy teams as well as connecting and advising therapy teams in the hospital you will enjoy networking. As well as support from your line manager you will be supported by the Head Occupational Therapist for Lambeth and connected to the wider Borough mental health occupational therapists through regular local forums. In addition the trust occupational therapy team provides training and regular opportunities for networking across the trust.
