Crynodeb
Deliver highly effective medical care to the registered population. Provide services in line with the Primary Care Contract. Clinically examine and assess patients physical and psychological needs and plan care accordingly, referring to other services when appropriate, using practice and local guidelines. Prescribe generically and safely in line with local and national guidance. Prioritise health problems and intervene appropriately in complex, urgent, or emergency situations, initiating effective emergency care when required. Effectively manage patients with long-term conditions. Promote health education and self-care, supporting patients to adopt healthier lifestyles. Process clinical administration in a timely way, including referrals, prescriptions, results, and correspondence. Review medications in line with NICE guidance, local pathways, and practice policy. Maintain accurate, high-quality clinical records. Share practice workload fairly and work collaboratively with the team. Follow best practice through clinical guidelines and audit. Support anticipatory and personalised care planning. Contribute to continuous improvement and quality initiatives. Take delegated responsibility for agreed areas, including aspects of QOF. Attend and contribute effectively to practice meetings. Support clinical governance, training, and service development. Follow safeguarding policies for adults and children at all times. Comply with appraisal and revalidation requirements. Maintain ongoing professional development and encourage learning across the team. Adhere to all practice protocols and policies. Promote a culture of teamwork, respect, and continuous improvement. Work as part of the wider primary care network and multidisciplinary team. Undertake other reasonable duties according to service needs and staffing levels. Undertake home visits when required in line with practice policy and service needs. Provide clinical supervision, guidance and support to other clinicians and members of the wider team, as appropriate to experience and role.
