Crynodeb
To provide high-quality, evidence-based nursing care to BCYP in their home, school, nursery, or other community settings, consistently working within the scope of competencies achieved as outlined in the Nursing Workbook and Competency Handbook for the Community Childrens Nursing Service. To work in partnership with the child, young person and their family and carers, to identify individual needs and deliver nursing care. To encourage and support self-management and independence of clients and families, providing training and support within limits of identified competence. Work alongside the nurses, nurse associates and students to develop nursing skills and nursing practice in order to be fully competent in their roles. To work as part of the Community Childrens Nursing Service to ensure the ongoing assessment, planning, implementation and evaluation of individualised evidence based care. Develop with other team members effective liaison with local statutory, voluntary and third sector organisations. Providing contribution to multidisciplinary assessments for children and young people. Attendance and contribution to multi-disciplinary, multi-agency meetings and discharge planning meetings. Participate in the education, training and monitoring required to meet care needs of children and young people in their home, and community settings including schools, nurseries.
