Summary
We are seeking an enthusiastic, forward-thinking and highly skilled Diabetes Specialist Nurse to join our Diabetes Service, delivering high-quality care across community and antenatal settings.
This role offers an excellent opportunity to be part of a transformational programme of work, with a strong emphasis on digital innovation, virtual care and the expanded use of diabetes technology to improve patient outcomes and experience.
This post is being offered as 1.6 FTE to be shared between the successful candidates. Part time or full-time hours will be considered.
• Provide expert clinical care and education to people with diabetes across community and antenatal pathways, using a blend of face-to-face and virtual consultations
• Lead and support the management of gestational diabetes and diabetes in pregnancy, working closely with midwifery, obstetric and medical teams
• Support people with Type 2 and gestational diabetes, including insulin initiation and optimisation
• Promote and support the use of diabetes technologies, including flash glucose monitoring, CGM and digital platforms
• Contribute to and help shape a transformational project focused on service redesign, digital delivery and improved access to care
• Deliver personalised care planning and self-management support
• Act as a clinical leader, contributing to service development, audit, and quality improvement
• Provide supervision, mentorship and education to junior staff and students
• Work autonomously while functioning effectively within a multidisciplinary team
